چکیده

Background Individuals with equivocal nuclear stress test results are at higher risk for cardiac events compared to those with normal studies. CMR stress perfusion imaging identifies individuals with coronary artery disease with high sensitivity and specificity, with excellent spatial resolution, and without ionizing radiation. The utility of CMR stress testing is uncertain in individuals with prior equivocal stress results.

Perfusion imaging with vasodilator stress echocardiography: a physiologically sound approach to coronary disease?

Cardiac stress testing with noninvasive imaging is increasingly used not only to detect the presence of coronary artery disease (CAD), but also to help to select appropriate therapy according to the extent and severity of disease. To that end, there have been continuous efforts to improve the performance of all forms of stress testing. For example, recent advances in stress radionuclide imaging...

Peri-infarct ischemia determined by cardiovascular magnetic resonance evaluation of myocardial viability and stress perfusion predicts future cardiovascular events in patients with severe ischemic cardiomyopathy.

BACKGROUND We assessed whether cardiovascular magnetic resonance imaging (CMR) of peri-infarct ischemia provides prognostic information in severe ischemic cardiomyopathy (ICM) patients referred for revascularization. METHODS Twenty-one patients with severe ICM were recruited prospectively for combined stress adenosine perfusion, late gadolinium enhancement, and rest perfusion studies. The pat...

Stress Perfusion Cardiac Magnetic Resonance Imaging Effectively Risk Stratifies Diabetic Patients With Suspected Myocardial Ischemia.

BACKGROUND Diabetics remain at high risk of cardiovascular disease and mortality despite advancements in medical therapy. Noninvasive cardiac risk profiling is often more difficult in diabetics owing to the prevalence of silent ischemia with unrecognized myocardial infarction, reduced exercise capacity, nondiagnostic electrocardiographic changes, and balanced ischemia from diffuse epicardial co...

Real-time cine and myocardial perfusion with treadmill exercise stress cardiovascular magnetic resonance in patients referred for stress SPECT

BACKGROUND To date, stress cardiovascular magnetic resonance (CMR) has relied on pharmacologic agents, and therefore lacked the physiologic information available only with exercise stress. METHODS 43 patients age 25 to 81 years underwent a treadmill stress test incorporating both Tc99m SPECT and CMR. After rest Tc99m SPECT imaging, patients underwent resting cine CMR. Patients then underwent ...

Regadenoson cardiovascular magnetic resonance myocardial perfusion imaging predicts need for future revascularization

Background Regadenoson (Lexiscan; Astellas) is a new vasodilator myocardial stress agent that selectively activates the A2A receptor. Unlike adenosine, regadenoson is easier to administer and results in fewer side effects. Although extensively studied in patients undergoing nuclear myocardial perfusion imaging (MPI), its performance in cardiovascular magnetic resonance (CMR) MPI remains unknown...
